Through our developmental questionnaire, The questionnaire is a great tool for parents that helps to reveal your child’s strengths, uncover any areas of concern and determine if there are community resources or services that may be useful for your child or your family.

Its simple! We mail you the questionnaire, you complete it with your child and send it back to us. We’ll call you to let you know how your child’s doing and provide activities that you can do with your child in the home. With your permission, we also send the results to your child’s pediatrician so that he or she are kept informed of your child’s progress.
